1. A description of the problem. (i.e. lost connection, slow speeds, high pings, etc.)
For the last year or so (out of 3 years at this location) I've had consistent issues with ping causing issues while gaming. I can stream HD video to my tv without issue, I can download large files without issue, but 90% of the time when gaming I will have consistent ping spikes causing lag in the game. This happens with Rocket League, World of Tanks and others, so it's not isolated to one game. Somedays it is a small lag spike causing little issue, other days it makes the game unplayable. I purchased my own cable modem to try fix it, and no luck. I purchased a second cable modem, in case it was a defective device (on the advice of Mediacom Support), no luck. Mediacom came out and put a splitter on my modem because my power levels were too high, which seemed to help at first, but the issue persisted. Then Mediacom came out and replaced the entire line from the street to my house, which had no effect. I'm running out of ideas. I don't do a lot of gaming so I've been living with it, but I'd like to find a solution.
2. Give a description of any home networking setup you may have. (i.e. router make/ model, hubs, adapters, etc.)
I have a Netgear CM500 modem, Firmware version V1.01.04 (also tried a Aris Surfboard SB 6141) connected to an Asus RT N66R Wireless router. There is an ethernet line running from the wireless router directly to my PC.

8. Is the problem constant, intermittent, or does it occur at a specific time of day? If it is an intermittent problem, a 24hr smokeping is recommended.
Occurs throughout the day, the severity changes day to day. I don't game every day, but over the year that I've been experiencing this I can say it happens at all times during the day and every day of the week.
9. Include any firewall and/or anti-virus software you have installed and what version you have.
Recently switched from ESET to Malwarebytes Premium (4.2.1) (no changes in behavior with the switch).
